Nevada prices its formation filing at the Secretary of State and posts the mail queue date it is working through, with expedited formation on offer. Member and organizer names reach the public formation record, so anonymity is marked unavailable here. The recurring charge is called the state business license and carries a published floor.
That license money is already inside the annual report figure, which is why the sheet counts it once and never twice. The fictitious firm name belongs to each county clerk rather than the state office, and it stands outside the entity name screen: it neither blocks nor is blocked by names on the state's records. A complete name check therefore means county records, not one statewide search.

Office or storefront in Nevada?
A business with a real footprint in Nevada files in Nevada. Forming anywhere else adds a foreign registration back home: two filings, two annual bills, two registered agents. The out-of-state discount does not survive that arithmetic.

field 05 carries the date it was observed, not a promise: of the two halves beside it, only the expedited tiers are prices the state commits to
Nevada calls it the fictitious firm name, and handles it at the county level, so the price and the paperwork are set locally rather than by the state.
5 years
A fictitious firm name certificate is effective for five years and is renewed by filing a new certificate with the county clerk.
not required
no exclusive right
A fictitious firm name is a county-level notice filing that discloses who is behind a business name; it grants no exclusive right. Because it is filed with the county clerk and not the Secretary of State, it is not part of the state entity-name distinguishability screen, so a fictitious firm name neither blocks nor is blocked by names on the Secretary of State's records.
Fictitious firm names are held in each county clerk's records, not in the Secretary of State's business search, so a full check means searching the clerk in every county where a business operates. The Secretary of State publishes a directory of county clerk contacts for this purpose.

Every figure on this sheet renders from the pinned dataset snapshot, and each card links the page its own figure came from. This list collects those pages in one place, with the duplicates removed and the agency named.
The rule behind every figure here, and what a card does when one cannot be verified, sits on how a figure gets verified.
